The attached document shows components directly required for a Level B assessment as part of EDUC 622- Psychoeducational Assessment instructed by Melanie Baerg. Included are a standardized self-assessment sheet highlighting my own perceived success in test administration, parental consent form with personal identification redacted, as well as the record form which the administered test was completed on, along with a score summary, final findings and anecdotal notes.
 
I have included the Kaufman Test of Educational Achievement 3rd Edition (KTEA-3) as a Practice artifact, as it had immediate relevance in my professional practice. Upon completion of EDUC 622, I became only the second staff member, out of nearly 50, at Godson Elementary to be certified in the administration of Level B Assessments. As a result, I have already officially administered KTEA-3 assessments with five different students, and have one who will be receiving a Level C assessment from a registered psychologist before the end of this school year, based upon my findings. This will result with them being recommended for a Ministry of Education Special Education designation, and eventually an Individualized Education Plan.
 
As such, this artifact has become extremely vital to my own practice, as well as the efficiency of our school to properly service students with undiagnosed learning difficulties.
